Warranty and Remake Policy

Effective Date: November 15, 2025

1. Scope and Key Terms

This Policy covers custom dental prosthesis such as crowns, bridges, dentures, implant components, occlusal splints, and printed try-ins or printed solutions. It also covers non-custom supplies such as stock parts and accessories. “Warranty Period” means the coverage periods in Section 3. Remedies are limited to repair, remake, replacement, or credit at Ivorymade’s option.

2. General Conditions

Provide the prescription or specifications, scans or models, occlusal records, shade information, and photos we reasonably request. If requested, return the original prosthesis and related models or alloy. Failure to return may void coverage under Section 7. Chair time and other clinical costs are excluded from any remedy. Accounts must be current at the time of claim.

3. Warranty Coverage and Periods

Ivorymade warrants that its custom dental prosthesis will be free from defects in materials and workmanship for the periods below, starting on the delivery date.

• Temporary crowns and bridges, and printed solutions: 60 days

• Zirconia restorations, including layered zirconia frameworks: 5 years

• Titanium abutments and titanium bars: 6 years

• Layered porcelain, and e.max or lithium-disilicate: 2 years

• All other fixed or removable prosthesis not listed above: 1 year

Remedies: Ivorymade will repair, remake, replace, or credit up to the price paid for the prosthesis, at Ivorymade’s discretion.

4. Remake or Repair for Custom Dental Prosthesis

At no charge during the applicable Warranty Period when a failure results from Ivorymade materials or workmanship, and required items are returned. Remake fees, according to the current fee schedule, may apply when the request results from any of the following:

• Shade, material, design, or treatment-plan changes

• Proceeding despite incomplete or unsatisfactory records, or declining a try-in where indicated

• A prosthesis that fits the model but not the mouth

• Failure to return the original prosthesis, models, or alloy when requested

5. Returns and Refunds for Non-custom Supplies

Returns and refunds do not apply to custom dental prosthesis. For non-custom supplies:

• Obtain a Return Merchandise Authorization (RMA) before shipping any return

• The return must be received within 30 days of RMA issuance

• Items must be in new condition with original packaging, and unopened where sterility applies

• Refunds are typically issued as account credit

• Original shipping charges are not refunded unless there is a verified defect or carrier issue

• Risk of loss remains with the sender until Ivorymade receives the item

6. Shipment or Billing Issues

Report shortages, transit damage, or billing disputes within 30 days of invoice or delivery, whichever is earlier, so Ivorymade can investigate and adjust if appropriate.

7. When Coverage Is Voided

Coverage under Sections 3 and 4 is void if any of the following apply:

• Failure to return the original prosthesis, models, or alloy when requested

• Alteration or repair by others without Ivorymade’s written consent

• Misuse, abuse, trauma, parafunction, poor hygiene, or failure of supporting teeth, bone, or tissues

• Use of non-indicated materials or designs contrary to instructions for use

• Account is past due at the time of claim

• Failure to provide reasonably requested records, photos, or scans necessary to evaluate the claim

8. Limitations of Liability and Implied Warranties

Your exclusive remedies are repair, remake, replacement, or credit of the price paid for the prosthesis or supply at issue. Consequential, special, or indirect damages, including chair time, are excluded to the maximum extent permitted by law. If an implied warranty may not be disclaimed, its duration is limited to 90 days.

9. Dispute Resolution, Governing Law, and Venue

This Policy incorporates by reference the binding arbitration and class action waiver provisions in Ivorymade’s Terms of Use. To the extent any claim falls outside those provisions, California law governs and exclusive venue lies in the state or federal courts located in San Francisco County, California. The parties waive jury trial to the extent permitted by law.

10. Administration

Ivorymade may update this Policy prospectively. The version and effective date above control.
